多因子生物標志物應(yīng)用于早期特異性診斷漿液性卵巢癌診斷

由于大多數(shù)患者是在晚期確診,卵巢癌仍然是最致命的婦科惡性腫瘤。沒有單一的生物標志物具有所需的靈敏度和特異性。

耶魯大學醫(yī)學院婦產(chǎn)科,哈佛大學布列根和婦女醫(yī)院(Brigham and Women’s Hospital,BWH),丹娜法伯癌癥研究院,約翰霍普金斯大學醫(yī)學院西德尼Kimmel綜合癌癥中心等,聯(lián)合開發(fā)了一組新的生物標志物,用于早期發(fā)現(xiàn)高級別漿液性卵巢癌。結(jié)果發(fā)表在最新一起婦產(chǎn)科腫瘤權(quán)威雜志:Gynecologic Oncology。 

臨床研究設(shè)計:

  1. 候選因子:12個:IL-1Ra, IL-6, Dkk-1, uPA, E-CAD, ErbB2, SLPI, HE4, CA125, LCN2,MSLN, and OPN

  2. 研究對象: 172病人(有婦科良性病變,或高級別漿液性卵巢腺癌等)

  3. 技術(shù)平臺: Ella Simple Plex (ProteinSimple)

  4. 結(jié)果:CA125, HE4, E-CAD, IL-6 四因子組成的panel具有特異性診斷漿液性卵巢癌的臨床價值。     

a)      卵巢癌病人versus無腫瘤對照組:特異性100%,陽性預(yù)測價值100%,靈敏度 86.4%

b)      卵巢癌病人versus婦科良性疾病對照組:特異性100%,陽性預(yù)測價值100%,靈敏度 87.2%

c)      卵巢癌病人versus無婦科良性疾病對照組:特異性100%,陽性預(yù)測價值100%,靈敏度 98.4%

d)      早期漿液性卵巢癌 Versus無腫瘤對照組:特異性95.7%,陽性預(yù)測價值88.9%,靈敏度 84.2%  

文中,作者特別注明選擇Ella simple plex進行本項研究的原因:

Our study is one such effort to identify a panel of multiple serum biomarkers which is not just sensitive and specific, but also time-efficient and cost-effective. Simple Plex™is a new multi-analyte immunoassay platform, which has been proven to have significant advantages over traditional approaches in terms of low volume requirements (2.5 to 25 μL), sensitivity, and reproducibility [34,35]. Simple Plex™ eliminates inconsistency in sample handling and  produces results in a short amount of time, thus providing results with minimal error and in timely fashion. Simplex Plex™ costs much less and would be more cost-efficient to analyze multiple biomarkers.
